Why do wisdom teeth need to be removed?

Wisdom teeth often need removal because most people’s mouths don’t have enough space for these third molars to erupt properly. When there’s insufficient room, wisdom teeth become impacted, potentially causing pain, infection, damage to adjacent teeth, cysts, or crowding. Early removal prevents these complications and is typically easier when done before problems develop.
Wisdom teeth are typically removed during the late teens to early twenties when the roots are not fully developed, making extraction easier and recovery faster. However, removal can be performed at any age if problems develop. Early evaluation around age 16-18 allows us to monitor development and recommend removal before complications arise.
Signs that wisdom teeth may need removal include pain or swelling in the back of your mouth, difficulty opening your jaw, bad breath or taste, bleeding gums around wisdom teeth, or crowding of other teeth. However, problems can develop without symptoms, which is why regular dental X-rays and examinations are important for early detection of potential issues.
Wisdom teeth can be impacted in several ways: soft tissue impaction (crown visible but gums cover part of the tooth), partial bony impaction (tooth partially erupted with some crown below gum and bone), and complete bony impaction (tooth entirely enclosed in jawbone). The type of impaction affects the complexity of removal and recovery time.
Modern wisdom tooth removal is much more comfortable than many patients expect. We use effective local anesthesia to numb the area completely, and sedation options are available for anxious patients. Post-operative discomfort varies by case complexity but is typically manageable with prescribed or over-the-counter medications and usually subsides within a few days.
We offer various anesthesia options including local anesthesia for simple extractions, nitrous oxide for relaxation, oral sedation for moderate anxiety, and IV sedation for complex cases or multiple extractions. The choice depends on the complexity of your case, your anxiety level, and personal preferences. We’ll discuss the best option for your comfort during consultation.
Simple wisdom tooth extractions typically take 15-30 minutes per tooth, while complex impacted teeth may take 45-60 minutes. If multiple teeth are being removed, the procedure may take 1-2 hours total. The time varies based on factors like impaction level, root development, and individual anatomy. Most patients are surprised by how quickly the procedure is completed.
Recovery typically involves 2-3 days of rest with gradually decreasing discomfort and swelling. Peak swelling usually occurs 48-72 hours after surgery, then gradually improves. Most patients return to normal activities within a week, though complete healing takes several weeks. Following post-operative instructions carefully promotes faster, more comfortable healing.
For the first few days, stick to soft foods like yogurt, pudding, smoothies, mashed potatoes, soup, and scrambled eggs. Avoid hard, crunchy, spicy, or very hot foods that could irritate the extraction sites. Gradually reintroduce normal foods as healing progresses. Staying hydrated and maintaining nutrition supports proper healing and recovery.
